If you are travelling abroad you will need to start thinking about your travel vaccinations 8 weeks before.

Our travel services offer advice on travel illnesses and a range of vaccinations. In order for you to prepare for travel, complete and return to the surgery the travel questionnaire form below at least 8 weeks before your intended date of travel.

Failure to do so, may mean we are unable to offer you an appointment to have your vaccinations.
