If possible please bring ID such as a passport, driving licence or birth certificate, and proof of address such as a utility bill or rental agreement. If you are unable to provide these documents please tell our receptionist.
To obtain your previous GP records without delay, we will need your NHS number (this has 10 digits and is not the same as your National Insurance number. You can obtain it from your current GP).
If you are registering a child you will be asked to provide evidence that you have parental responsibility, we would expect the person with responsibility to be registered with the child.(Section 11 of the Children Act 2004 places a statutory duty on the NHS to safeguard and promote the welfare of children)

NHS Patients Resident Elsewhere in the UK (Temporary Residents)
Patients visiting from other parts of the UK may be seen as ‘Temporary residents’. You will be asked to complete a form at the practice which includes details about your usual registered GP so that we can liaise with them about your care if necessary. Please tell the reception team how long you expect to be in our practice area.

Visitors From Abroad Not Usually Resident in the UK
If you are not normally resident in the UK, you may wish to be seen as a Temporary patient. Alternatively you can be seen on a Private Patient basis, fees will be payable. You may be able to claim back the fees from your travel insurance. You will be supplied with a receipt. Prescriptions and any subsequent tests or X-rays will also incur additional fees.
We can only accept new patients who are within our inner boundary area. Patients can register as new patients within the ‘inner boundary’ and existing patients who move within the ‘outer boundary’ may remain registered at the surgery, as specified on the following map.

All Patients - Locations Outside our Area
If you are moving outside our area
GPs work together with District Nurses, Community Mental Care Nurses, and Terminal Care Nurses. For this reason we need to be strict about the area covered.
We cannot, therefore, take on new patients, or retain existing patients, outside our stated area. If you move, even slightly outside our area, you must register with a practice in your new catchment area. Existing patients who move within the ‘outer boundary’ may remain registered at the surgery, as specified on the outer boundary map below.
