| To register as a patient |
1. Ensure your place of residence falls within our catchment area (see map below).
2. For registration you require your NHS Medical Card which contains your unique NHS Number and details of your current GP.
3. If you are not in possession of an NHS medical card, you will need to complete a GMS 1 form which can be downloaded here - Form 1
You will also be required to provide extra identification to support your application – either a National Insurance Card, Birth Certificate or Passport. You will also need your unique NHS Number which you can obtain from your present GP.
4. In all cases please download and complete our health questionnaire - Form 2
5. You are required to attend the surgery in person to register.
