| Medical Certificates |
If you are unable to work because of sickness, you can provide a self certificate for your employers for the first week of absence. The form is called an SC2, it can be obtained from the surgery , please ask at reception, you do not need to see a Doctor for this purpose. Most employers also have supplies of these too.
If your employer asks you to obtain a medical certificate from your Doctor for the first week of your sickness, please download this letter
